Only submit this form once. If you should wish to select a different first or second choice after submitting your application, please call our PUSD Pathways Office at 559-793-2437 and make the request BEFORE the January 15th application deadline.

Students may also download a hard-copy of the Pathways Application in English/Spanish. Please return any paper applications to either the PUSD Pathways Office (600 W. Grand Ave.) or to your middle school front office.
